FINGERPRINTING DATE AT OCEAN VIEW FOR VOLUNTEERS—OCT 15 (8:15 AM – 10:00 AM)

1) Complete a Volunteer Application
2) Bring your completed TB test results to the Ocean View Office at the time of fingerprinting, along with application.
2) Pick up a Lifescan form to complete so that you may be reimbursed up to $20; you must submit a current TB Test first.
3) Bring a photo ID (e.g. Driver's License) or Passport to show at time of fingerprinting.
