FINGERPRINTING DATE AT OCEAN VIEW FOR VOLUNTEER II OR DRIVERS—WEDNESDAY, SEPTEMBER 27TH (8:15AM– 9:45 AM), ROOM #15:
1) Complete a Volunteer II Application Online (AUSD website, under “Employment” tab, then “Volunteering). Scroll to Volunteer II, and click on “Click Here to Apply”.
2) Bring completed/signed by doctor TB Risk Assessment Questionnaire (blanks available in OV Office) OR TB test results from medical office/clinic to the Ocean View Office at the time of fingerprinting, along with Volunteer II Application (if can be printed out) . (Fingerprinting can’t be done until these forms received)
3) Pick up a Lifescan Form in the school office, to complete, after showing other completed forms mentioned above.
4) Bring a Photo ID (e.g. Driver's License) or Passport to show at time of fingerprinting.
